Short Term Leave Request.
This form is for ALL visiting and permanent professors. If you are absent
for more than 7 working days during the quarter, you must submit this
form to the department chair. After the department chair has signed it,
please submit the form to Claudia Carlson. Submitting this form before
you leave on business helps to secure Worker's Compensation benefits should
you have an accident.
Entertainment Reimbursement

Fill in all you can and submit all copies and your receipts to Denna Zamarron.

Fill in all you can and submit all copies and your receipts to Denna Zamarron.
VISITOR
Form
For all visitors and seminar speakers who are to be reimbursed for their
expenses. Form can be filled electronically.
DEPARTMENT
Travel Reimbursement Worksheet
Department members must fill in the worksheet to receive reimbursement
for travel (this can be done electronically). Attach receipts and submit
to Denna Zamarron.
Hourly
Timecard. A monthly timecard must be submitted by all hourly employees.
This includes Research Assistants, graduate students working on Instructional
Improvement Grants, Lead TA Training Coordinators and Work Study Students.
Teaching Assistants do not have to submit a time card.
Bio-Bibliographic Update
in Microsoft Word format. This is a blank template that can be used to
update ladder faculty biobibs. This is due each year by May 10.
Biography Form for Academic
Personnel in PDF format. This form is used for all new permanent and
visiting faculty including student associates. The form can be filled
in online and printed out, however, an original signature is required.
A current C.V. must be attached.
TA Assignment Form
(for UAW union requirements). This form is in PDF format and can be filled
in online and printed out. This form must be completed by the Instructor
for each Teaching Assistant and Reader before the quarter begins (or before
they perform their assignments). This includes Summer Session.
Conflict of Commitment Form.
All faculty members are required to fill in Appendix C with category I
and II outside activities. at the end of each academic year. If there
are no such activities write "NONE". The form is due by November
1 of the following year. If there will be category I activities, prior
approval from the Chancellor is required, therefore you must fill in Appendix
B and receive approval prior to the activity. See
APM 025.
